Membership Applications

 

The applications for membership (for all classes of members) are found below.  Please take a moment download, fill out the application, and submit it by fax, e-mail, or regular mail.

This year, as a convenience to our members, we are accepting credit card payments.  The instructions for payment by credit card are attached to the application.

2008 Regular Member Application

This application is for the chief and all chief officers of a fire department or other agencies and organizations eligible for regular membership in the association.

2008 Individual Member Application

This application is for other individuals eligible for active membership according to the Constitution and Bylaws such as county or municipal emergency managers, county fire marshals, the fire director of federal wildland fire agencies based in Colorado, etc.


2008 Associate Member Application

This application is for individuals who work in allied fields of the fire service and are interested in the goals and objectives of the Association, and are not identified with fire protection commercially.


2007-08 Corporate Sponsor Application

This application is for individuals and/or businesses engaged in the manufacture or sale or emergency apparatus, equipment, supplies or service, or provide products and/or services to fire departments and support the goals and objectives of the Association.

About Banner Ads

 

The banners used in banner ads are to be no larger than 468 x 97 pixels.  Typical banners include a company logo, company name, and three or four lines of text depending on remaining space. Recommended file size approximately 18K - 30K (72 dpi)
